Established patients can request an appointment by clicking the 'Request an appointment' button in the upper right of this page. This is only to request an appointment, the date and time you pick may not be available or appropriate for your medical needs. We will either accept your request or reappoint into another date and/or time. We will notify you of this via email or phone call. Please make sure your email is correct. COVID-19 UPDATE (1 Nov 2023): Patients are encouraged to wear masks in the clinic, especially during the winter months. The doctor and techinicians will be masked while providing direct patient care in the clinic. Our hours are Monday through Friday from 10:30 AM to 5:00 PM.
